NOTE: You Must upload your PI's approval email listing the EXACT ROOM NUMBERS they approve you to receive keys for.

Key Request Form

1. Complete the key request form.
2. Upload your "Laboratory Safety Fundamentals" certificate if requesting keys for a lab space.
3. Non PI's need to upload PI approval for all key requests.
4. Submit request.
5. Once you have submitted the request, you will receive an email to schedule a key pick-up appointment.
